Vaccination History

The Practice can issue information concerning your vaccination history. As your vaccination status/history is very important, your records require to be checked by a clinician. To obtain your vaccination history please attend the practice to complete Data Protection forms indicating the information that you require. Please allow at least seven to ten working days for your requested information to be available. The Practice will contact you when this is ready for collection. Photographic ID will have to be viewed before release of the information.
